Health GUIDE
School & sports physicals
A school or sports physical reviews health and participation requirements. Osita provides examinations for school entry and sports, including a health-history review, vital signs, vision screening, and immunization review as appropriate.
General patient education. Your clinician’s individual instructions take priority. An online guide cannot determine eligibility or replace an examination.
Before your appointment
- Bring the school or sports form, immunization records, glasses or contacts, medication list, and any relevant medical records. Ask about minor consent requirements and same-day availability when scheduling.
- Bring identification, insurance details, medicines, allergies, relevant records and any school, sports or employer forms.
- Confirm whether your particular test needs fasting or other preparation. Do not fast or stop medication unless instructed. For surgery clearance, bring your surgeon’s requirements.
What to expect
Your provider examines you and reviews the required form. Additional testing or follow-up may be needed before the form can be completed. Clearance is based on the assessment and is not automatic.
After your visit
- Before leaving, confirm the next steps and how any results, forms or referrals will be communicated.
- Contact the clinic if the expected follow-up has not arrived. Do not assume that no message means every result is normal.
Follow-up & what to expect
The assessment may result in clearance, restrictions or a request for further evaluation. Allow time before the school or team deadline; same-day form completion cannot be promised.
Your questions, answered
What forms should I bring?
Bring the exact school or league form with the history section completed, immunization records, glasses or contacts, and relevant medical records. Confirm whether a parent or guardian must attend.
Is a sports physical the same as a full annual checkup?
No. A participation examination focuses on activity safety and required documentation. Preventive care and other health concerns may need a separate visit.
What history matters for sports?
Tell the provider about fainting, chest pain with exertion, breathing problems, concussions, injuries and family history of early sudden death or heart disease.
Is clearance automatic if I feel healthy?
No. Symptoms or findings may need further assessment. Do not continue strenuous sport against medical advice while clearance is pending.
Are vaccines or extra tests included?
Ask beforehand. Requirements, availability and costs differ; the physical appointment alone does not guarantee every required service is included.
Planning your visit
What will it cost?
Confirm visit fees and what is included before scheduling. Tests, medication, supplies or additional visits may be separate. Ask about insurance eligibility for medical care; coverage is not guaranteed. A consultation is not advertised as free.
How can I contact the clinic?
