Health GUIDE
Work physicals
Work physicals assess health in relation to an employer’s or program’s requirements. The examination and documentation depend on the duties and the specific form requested.
General patient education. Your clinician’s individual instructions take priority. An online guide cannot determine eligibility or replace an examination.
Before your appointment
- Bring identification, employer instructions, required forms, and your medication list. Confirm any specialized certification or test requirement with the clinic before booking.
- Bring identification, insurance details, medicines, allergies, relevant records and any school, sports or employer forms.
- Confirm whether your particular test needs fasting or other preparation. Do not fast or stop medication unless instructed. For surgery clearance, bring your surgeon’s requirements.
What to expect
The visit may include vital signs, vision screening, a medical-history review, and an examination. If additional tests or TB screening are required, the clinic will explain the next steps and timing.
After your visit
- Before leaving, confirm the next steps and how any results, forms or referrals will be communicated.
- Contact the clinic if the expected follow-up has not arrived. Do not assume that no message means every result is normal.
Follow-up & what to expect
Keep a copy of completed documents and confirm the employer receives the requested information through an agreed route. Additional testing or review may delay completion.
Your questions, answered
Can this meet any employer’s requirements?
Not necessarily. Send or describe the exact requirements before booking. Specialized certifications or regulated examinations may need a specifically authorized examiner.
Is this a DOT physical?
Do not assume so. Confirm availability of the exact certification and qualified examiner with the clinic before scheduling.
Can I add TB or drug testing?
Ask which tests are available and whether separate orders or appointments are required. Some tests need a return visit or laboratory processing.
What if I use glasses or prescribed medicine?
Bring corrective lenses and a current medication list. Explain any work restrictions or relevant conditions so the examination can address the stated duties.
Who gets my results?
Confirm what documentation the requesting organization requires and how it will be released. Do not put private employment or medical records into the general inquiry form.
Planning your visit
What will it cost?
Confirm visit fees and what is included before scheduling. Tests, medication, supplies or additional visits may be separate. Ask about insurance eligibility for medical care; coverage is not guaranteed. A consultation is not advertised as free.
How can I contact the clinic?

For severe or rapidly worsening symptoms, get medical care promptly. Call 911 for an emergency. The website form is not an urgent-care service.
